ET is commonly viewed as a relatively benign disease.<\/p>\n\n\n\n<p>However, the associated disabling aspects of ET, such as significant tremor of the hands, can impair patients\u2019 ability to eat, shave, write, perform household activities, and function in the workplace.<\/p>\n\n\n\n<p>The underlying etiology of ET is unclear, but cases often run in families (familial tremor). It has been estimated that approximately 50% of all cases are due to a genetic mutation, and the pattern of inheritance is most consistent with autosomal dominant transmission (i.e. dominant trait).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-focused-ultrasound-therapy\">Focused Ultrasound Therapy<\/h2>\n\n\n\n<p>Focused ultrasound is a noninvasive, therapeutic technology with the potential to improve the quality of life and decrease the cost of care for patients with essential tremor. This novel technology focuses multiple beams of ultrasound energy precisely and accurately on targets deep in the brain without damaging surrounding normal tissue.<\/p>\n\n\n\n<p><strong>How it Works<\/strong><br>Where the focused ultrasound beams converge, they produce precise ablation (thermal destruction of tissue), enabling ET to be treated noninvasively. The primary target is a region in the thalamus called the ventral intermediate nucleus (Vim), but other adjacent targets and white matter pathways are also in use.<\/p>\n\n\n\n<figure class=\"wp-block-embed alignright is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"A Focused Ultrasound Patient Story: Rick (Essential Tremor)\" width=\"640\" height=\"360\" src=\"https:\/\/www.youtube.com\/embed\/OMzguVx5USs?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<p><strong>Advantages<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>It is a noninvasive, single treatment that enables patients to recover rapidly and quickly return to activities of normal life (usually the next day).<\/li>\n\n\n\n<li>Compared to RF ablation or DBS, focused ultrasound offers a reduced risk of infection, of damage to the non-targeted area, and of blood clot formation.<\/li>\n\n\n\n<li>Focused ultrasound offers rapid resolution of symptoms.<\/li>\n\n\n\n<li>In contrast to lesioning performed with stereotactic radiosurgery, focused ultrasound does not use ionizing radiation, thus avoiding the side effects of exposure to radiation.<\/li>\n\n\n\n<li>Because it is non-invasive, focused ultrasound could be an option for medically refractory ET patients (those who do not respond well to medication) who do not want to undergo surgery.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-clinical-trials\">Clinical Trials<\/h2>\n\n\n\n<p>A\u00a0<a href=\"https:\/\/clinicaltrials.gov\/search?term=NCT07417280\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>\u00a0in Toronto, Ontario, Canada is using low intensity focused ultrasound to impact patients with multiple types of disorders, including many types of tremors, dystonia, epilepsy, and addictive substance abuse conditions.\u00a0\u00a0<\/p>\n\n\n\n<p>A&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06974916?cond=NCT06974916&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>&nbsp;in Europe&nbsp;is using&nbsp;focused ultrasound to treat patients with bilateral&nbsp;tremor.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A <strong><a href=\"https:\/\/clinicaltrials.gov\/search?term=NCT06331052\">clinical trial<\/a><\/strong> is recruiting patient for 3-D tractography targeting for essential tremor.<\/p>\n\n\n\n<p>A <strong><a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05475340?term=focused%20ultrasound&amp;cond=Essential%20Tremor&amp;aggFilters=status:not%20rec&amp;rank=2\" class=\"ek-link\">clinical trial<\/a><\/strong> is using transcranial ultrasound to treat essential tremor in the USA.<\/p>\n\n\n\n<p>There is a&nbsp;<strong><a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05965960?term=NCT05965960&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\">clinical trial<\/a><\/strong> in Toronto Western Hospital recording intracranial measurement for patients with treatment for movement disorders.<\/p>\n\n\n\n<p>There is a second&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07212244?term=NCT07212244&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a>&nbsp;in Toronto, evaluating the clinical outcomes for patients treated&nbsp;for&nbsp;essential&nbsp;tremor.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07172295?term=NCT07172295&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a>&nbsp;in Taiwan is treating patients with refractory essential tremor, Parkinson\u2019s disease tremor or dystonia using focused ultrasound.&nbsp;&nbsp;<\/p>\n\n\n\n<p>There is a <strong><a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05624385?term=focused%20ultrasound&amp;cond=Essential%20Tremor&amp;aggFilters=status:rec&amp;rank=5\">clinical trial<\/a><\/strong> in China for patients with a reduced SDR.<\/p>\n\n\n\n<p>The Foundation updates these pages regularly, but with the increasing number of clinical trials, we want to be sure that our audience has the latest information available. It is also approved for treating essential tremor in Europe, Korea, Canada, Japan, Russia, Taiwan, and the Middle East.&nbsp;<\/p>\n\n\n\n<p>The <a aria-label=\"FDA approved staged bilateral treatment (opens in a new tab)\" href=\"https:\/\/insightec.com\/insightec-receives-fda-approval-to-treat-essential-tremor-patients-second-side-expanding-total-available-market-in-united-states\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"ek-link\"><strong><span style=\"text-decoration: underline\" class=\"ek-underline\">FDA approved staged bilateral treatment<\/span><\/strong><\/a> for essential tremor on December 20, 2022. Treatments are approved for the second side at least 9 months after the initial treatment.<br><br>In the US, all treatment sites have been approved for payment under Medicare.<\/p>\n\n\n\n<p>Commercial coverage and Medicaid will typically follow Medicare but should be evaluated with each company and region. Some facilities have a cash payment option, for those who are not willing to wait for insurance decisions for payment.
