This novel technology focuses beams of ultrasonic energy precisely and accurately on targets deep in the brain without damaging surrounding normal tissue.<\/p>\n\n\n\n<p>Please note that for work being done on dementia, including that which is associated with Parkinson\u2019s disease, please see the Disease and Conditions listing for <a class=\"ek-link\" href=\"\/diseases-and-conditions\/dementia\/\"><strong>Dementia<\/strong><\/a>.<\/p>\n\n\n\n<p><strong>How it Works<\/strong><br>Where the beams converge, focused ultrasound produces a variety of therapeutic effects enabling Parkinson\u2019s disease to be treated without surgery. For movement disorders, the mechanism of action is ablation (thermal disruption of the tissue) that can be aimed at several different treatment targets. For the preclinical work that one day may treat the underlying causes for Parkinson\u2019s, the mechanisms can be temporarily disrupting the blood-brain barrier (BBB), which will allow desired therapeutics the ability to enter the brain. Opening the BBB also can enable undesired materials to more easily depart from the brain.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-benefits\">Benefits<\/h2>\n\n\n\n<p>Currently, there is no cure for Parkinson\u2019s disease (PD). Treatment options include drug therapies and in select patients, surgeries such as deep brain stimulation or radiofrequency ablation. Focused ultrasound has the ability to noninvasively treat some Parkinson\u2019s disease patients through thermal ablation of the brain circuits that cause motor symptoms. Focused ultrasound can also open the blood-brain barrier (BBB), a naturally occurring barrier of tightly aligned cells along the blood vessels, allowing the diffusion of medications and antibodies into the brain that can potentially prevent progression and\/or restore function.<\/p>\n\n\n\n<p>Advantages:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Noninvasive &#8211; no incisions, no risk of infection or bleeding, less pain, and rapid recovery<\/li>\n\n\n\n<li>Image-guided &#8211; precision targeting with minimal damage to surrounding tissue<\/li>\n\n\n\n<li>Safe, temporary and repetitive opening of the blood-brain barrier (BBB) &#8211; enhancing the delivery of therapeutics and immune cells directly to the brain target site<\/li>\n<\/ul>\n\n\n\n<p><strong>Relief of Motor Symptoms<\/strong><br>The FDA has approved the use of focused ultrasound for treatment of tremor-dominated Parkinson\u2019s disease and Parkinson\u2019s dyskinesia, which is a broad definition that includes a wide variety of motor complications, including motor fluctuations, involuntary extremity movement, rigidity, slow initiation of movement and other similar diseases. The latest approval is for the treatment of the pallidothalamic tract (PTT), and this also includes the possibility of a staged (6 month interval) bilateral treatment.\u202f\u00a0Given the multiple treatment targets in the brain and the wide variety of the symptoms that are present with the movement disorders associated with Parkinson\u2019s disease, the manufacturer of the device (Insightec) suggests that medically diagnosed patients which are not controlled medically can either take this\u00a0<a href=\"https:\/\/insightec.com\/lp\/essential-tremor-quiz-reskin\/\" target=\"_blank\" rel=\"noreferrer noopener\">short quiz<\/a>\u00a0or contact Insightec (1-800-704-6797) for recommendations regarding next steps. \u00a0<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Parkinsonian tremor \u2013 target in the thalamus (thalamotomy)<\/li>\n\n\n\n<li>Parkinsonian dyskinesia \u2013 target in the globus pallidus (pallidotomy) or subthalamic nucleus<\/li>\n\n\n\n<li>Parkinsonian tremor, akinesia or dyskinesia \u2013 target in the pallidothalamic tract<\/li>\n<\/ul>\n\n\n\n<p>Additional Advantages<\/p>\n\n\n\n<figure class=\"wp-block-embed alignright is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"A Focused Ultrasound Patient Story: Kimberly (Parkinson&#039;s Disease)\" width=\"640\" height=\"360\" src=\"https:\/\/www.youtube.com\/embed\/272TzaUXg_U?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Focused ultrasound is non-invasive \u2013 no incisions, holes in the skull, electrodes in the brain \u2013 and therefore has reduced risk for infection and blood clots.<\/li>\n\n\n\n<li>Precise targeting minimizes damage to non-targeted healthy brain.<\/li>\n\n\n\n<li>Compared to deep brain stimulation, focused ultrasound is a single procedure, and does not require subsequent procedures\/visits to replace batteries, repair broken wires, or adjust simulator settings. It also does not involve the collateral damage to healthy tissue or the risk of infections associated with implanting a foreign body inside the brain.<\/li>\n<\/ul>\n\n\n\n<p><strong>Treating Underlying Pathology<\/strong><br>Preclinical studies suggest focused ultrasound\u2019s potential to restore function in Parkinson\u2019s models. Focused ultrasound can temporarily open the blood-brain barrier (BBB) to improve the delivery of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Genes, growth factors, stem cells, other neuroprotective and\/or neurorestorative drugs<\/li>\n\n\n\n<li>Anti-alpha synuclein antibodies<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-embed alignright is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-4-3 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"InSightec Focused Ultrasound Brain Treatment Animation\" width=\"640\" height=\"480\" src=\"https:\/\/www.youtube.com\/embed\/XyVdc6OeShY?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<p>In October 2018, the Foundation had a multi-disciplinary workshop that aimed at using BBB opening in conjunction with potentially curative agents, to decide what research approaches have the most potential. <a href=\"http:\/\/www.fusfoundation.org\/images\/pdf\/Focused-Ultrasound-Foundation-Parkinsons-Disease-BBB-Workshop-2018-Report.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Read the workshop summary &gt;<\/strong><\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-regulatory-authorizations\">Regulatory Authorizations<\/h2>\n\n\n\n<p id=\"h-regulatory-authorizations\">The Exablate system manufactured by&nbsp;<a aria-label=\" (opens in a new tab)\" href=\"https:\/\/insightec.com\/exablate-neuro\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"ek-link\"><strong>Insightec<\/strong><\/a>&nbsp;is approved in Europe and in the US for treating tremor-dominant Parkinson&#8217;s disease. In November 2021, it also earned approval in the US for treating additional movement disorders associated with Parkinson&#8217;s disease, including mobility, rigidity and dyskinesia. We recommend that patients talk with the treatment site for questions about insurance coverage under Medicare, as some treatment sites are now being reimbursed by Medicare.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-fda-approved-nbsp\">FDA Approved&nbsp;<\/h2>\n\n\n\n<p>The US FDA has recently cleared the use of focused ultrasound targeting the pallidothalamic tract (PTT) for treatment of movement disorder symptoms. This approval also includes a staged bilateral treatment, where the second side can be treated 6 months after the treatment of the first side. This adds to the prior approval of the unilateral ventral intermediate nucleus (VIM) target for tremor and the unilateral globus pallidus (GPi) target for mobility, rigidity and dyskinesia symptoms. &nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-clinical-trials\">Clinical Trials<\/h2>\n\n\n\n<p>There are several recent trials that assess the feasibility, safety, and preliminary efficacy of focused ultrasound to treat different symptoms of Parkinson\u2019s.&nbsp;<\/p>\n\n\n\n<p>A&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07207122?term=NCT07207122%20&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>&nbsp;in&nbsp;California is looking to see if&nbsp;transcranial&nbsp;ultrasound&nbsp;stimulation improves&nbsp;neurologic and&nbsp;cognitive&nbsp;outcomes&nbsp;in patients treated with neurodegenerative diseases.<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06763692?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> is organizing in West Virginia that will use low intensity focused ultrasound neuromodulation to treat patients with movement disorders from Parkinson\u2019s disease.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A\u202f<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05475340?term=NCT05475340&amp;draw=2&amp;rank=1%22%20\\t%20%22_blank\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>\u202fin Los Angeles is using transcranial ultrasound neuromodulation to treat patients with essential tremor.&nbsp;<\/p>\n\n\n\n<p>Two registries for <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT04991831?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=10\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>tremor<\/strong><\/a> and <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05539196?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=11\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>pallidotomy<\/strong><\/a> are looking at using MRI guided focused ultrasound to treat patients with movement disorders from Parkinson\u2019s disease.&nbsp;<\/p>\n\n\n\n<p>A&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/search?term=NCT07284719\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>&nbsp;in&nbsp;Denmark is trying to&nbsp;identify&nbsp;sustained biomarkers of adequate dopamine response after FUS treatments.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07172295?term=NCT07172295&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>&nbsp;in Taiwan is treating patients with refractory essential tremor, Parkinson\u2019s disease tremor or dystonia using focused ultrasound.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A\u202f<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT04593875?cond=Parkinson%27s%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:rec&amp;rank=2%22%20\\t%20%22_blank\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a>\u202fin Los Angeles is using Low Intensity Focused Ultrasound Pulsation (LIFUP) to treat patients with Parkinson\u2019s disease movement disorders has been completed. \u202f&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07044973?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=2\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> is recruiting patients in China and will treat with either the VIM or PTT target for movement disorders from Parkinson\u2019s disease.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A<strong> <\/strong><a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06639945?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=4\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a><strong> <\/strong>is recruiting patients in Shanghai, China and is using MRI guided focused ultrasound to treat patients with movement disorders.<strong>&nbsp;<\/strong><strong><\/strong>&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT04996992?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=6\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a><strong> <\/strong>is recruiting patients in Beijing, China to treat movement disorders from Parkinson\u2019s disease with focused ultrasound.<strong>&nbsp;<\/strong><strong><\/strong>&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05624385?cond=Parkinson%20Disease&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=13\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a><strong> <\/strong>in China is looking at using focused ultrasound to treat patient who have a low skull density ratio (SDR) value.&nbsp;<\/p>\n\n\n\n<p><strong><strong>Early-stage Parkinson\u2019s Disease<\/strong><\/strong><br>There is a <a class=\"ek-link\" href=\"https:\/\/clinicaltrials.gov\/study\/NCT06584383?term=NCT06584383&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a> in Spain for patients with early-stage Parkinson\u2019s disease, and the treatment will be randomized and include either unilateral subthalamotomy or best medical treatment.&nbsp;&nbsp;<\/p>\n\n\n\n<p><strong>Parkinson\u2019s disease patients with glucocerebrosidase deficiency<br><\/strong><a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05565443?term=NCT05565443&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>This trial<\/strong><\/a> is recruiting patients for treatment with recombinant glucocerebrosidase to offset this deficiency.
