Using real-time image guidance, the physician directs a focused beam of ultrasound energy to a selected volume in the patient\u2019s prostate gland.<\/p>\n\n\n\n<p><strong>How it Works<\/strong><br>Focused ultrasound energy heats and destroys the targeted tissue at the focal point within seconds. This process is repeated until the entire selected volume or the entire gland is destroyed.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-benefits\">Benefits<\/h2>\n\n\n\n<p>Current treatment options for prostate cancer include surgery, radiation, cryotherapy, hormone therapy, and chemotherapy. These treatment options carry risks and unwanted side effects such as urinary incontinence and erectile dysfunction. Focused ultrasound offers a treatment to patients with prostate cancer that carries fewer risks than conventional therapies. It may also be offered for patients with earlier stage disease where surgery is not yet recommended or as a salvage treatment for patients in which radiation has failed.<\/p>\n\n\n\n<p>Advantages:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Noninvasive &#8211; no incisions, no risk of infection or bleeding, less pain, and rapid recovery<\/li>\n\n\n\n<li>Image-guided &#8211; precision targeting with minimal damage to surrounding healthy tissue<\/li>\n\n\n\n<li>No ionizing radiation &#8211; fewer side effects and can be safely repeated<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-regulatory-authorizations\">Regulatory Authorizations<\/h2>\n\n\n\n<p>Focused ultrasound devices have been cleared to treat the prostate in approximately 50 countries, including the United States. Several systems for focused ultrasound ablation of prostate tissue are available in various geographical regions, each differing in their guidance method (ultrasound vs. magnetic resonance imaging) and approach \u2013 transrectal or transurethral.<\/p>\n\n\n\n<figure class=\"wp-block-embed alignright is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-4-3 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"Sonablate HIFU Video: 720x1280 - wide screen\" width=\"640\" height=\"480\" src=\"https:\/\/www.youtube.com\/embed\/5pTC7XdOPuw?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<p>While the Foundation attempts to keep the list of sites complete, there have been many purchases of the focused ultrasound systems. For the latest information on availability, we recommend contacting the manufacturers\u2019 directly or using their online search functions.<a href=\"https:\/\/www.fusfoundation.org\/the-technology\/treatment-sites\/treatmentsites\/list?indication_filter=Prostate%20cancer&amp;country=All%20Countries\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"ek-link\">&nbsp;<\/a><\/p>\n\n\n\n<p>There are four focused ultrasound manufacturers that are approved in the US for the ablation (destruction) of prostate tissue \u2013&nbsp;<a href=\"https:\/\/sonacaremedical.com\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Sonablate Corp.<\/strong><\/a>,&nbsp;&nbsp;<a href=\"https:\/\/www.edap-tms.com\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>EDAP-TMS<\/strong><\/a>,&nbsp;<a href=\"https:\/\/profoundmedical.com\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Profound Medical<\/strong><\/a>, and&nbsp;<a href=\"https:\/\/insightec.com\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Insightec<\/strong><\/a>. Sonablate and EDAP deliver care outside of the MRI facility, so they are found in surgery centers and surgical suites, as well as in hospitals.&nbsp;<\/p>\n\n\n\n<p>There are additional sites that use&nbsp;<em>mobile<\/em>&nbsp;focused ultrasound equipment. These sites may not be listed on the EDAP, Sonablate, or Profound Medical sites, and the best way to find them is to contact local urologists to see if they have focused ultrasound capability.<\/p>\n\n\n\n<p>Reimbursement for primary treatment in the US has been lagging, and Medicare and most insurance companies are either covering a portion of the expense or not covering it at all. In the specific instance of salvage therapy after failed radiation, some Medicare providers and commercial insurance carriers are covering focused ultrasound use in this setting. As always, it is best to check with your specific carrier.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-clinical-trials\">Clinical Trials<\/h2>\n\n\n\n<p>The PRISM&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07184970?term=NCT07184970&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a>&nbsp;in New York is&nbsp;investigating&nbsp;the rates of recurrence after HIFU treatment.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A second&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/study\/NCT07203482?term=NCT07203482&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical&nbsp;<\/strong><strong>trial<\/strong><\/a>&nbsp;in New York is evaluating&nbsp;clinical&nbsp;outcome&nbsp;for patients that have had HIFU treatment.&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06402357?term=NCT06402357&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>clinical trial<\/strong><\/a> in Florida is looking at patients quality of life outcomes, with localized radiorecurrent prostate cancer when treated with a the Focal One device.<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06178354?cond=Prostate%20Cancer&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=2\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> in California is comparing treatment of prostate cancer with thermal ablation or with cryotherapy.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT06270043?cond=Prostate%20Cancer&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=3\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> in Southern California is comparing the outcomes for different focal therapy devices for treatment of prostate cancer.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>A\u202f<a class=\"ek-link\" href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05001477?term=05001477&amp;draw=2&amp;rank=1\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>registry for the TULSA-PRO\u202f<\/strong><\/a>is tracking patient outcomes for this device.<\/p>\n\n\n\n<p>A\u202fmulti-site\u202f<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05027477?term=NCT05027477&amp;draw=2&amp;rank=1%22%20\\t%20%22_blank\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a>\u202fin the US, Canada and Finland is completing a randomized trial comparing the TULSA PRO to radical prostatectomy.\u202f\u202f&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT05610852?cond=Prostate%20Cancer&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=6\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> in Ohio is comparing a single port robotic, partial prostatectomy to focused ultrasound in patients with prostate cancer.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT03620786?cond=Prostate%20Cancer&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=10\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> in Southern California is evaluating quality of life questionnaires for patients that have had Sonablate treatments for prostate cancer.&nbsp;&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT04808427?cond=Prostate%20Cancer&amp;term=focused%20ultrasound&amp;aggFilters=status:not%20rec&amp;rank=11\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><\/strong><strong>clinical trial<\/strong><\/a> in Bethesda, Maryland is evaluating patients who are having the Tulsa-Pro treatment for low volume and low grade prostate
